Continuing your Orthopedic / Spine Care

Patients are encouraged to establish care with a new orthopedic surgeon or spine specialist as soon as possible to avoid any disruption in treatment.

Ways to find a new provider:

• Contact your primary care physician/provider (PCP) for a referral

• Check with your health insurance provider for in-network orthopedic or spine specialists

• Consider local healthcare systems or hospitals that offer orthopedic and spine care and/or provide physican referral services

Accessing your Medical Records

You may obtain a copy of your medical records related to your care with Dr. Kaan (e.g. clinical notes) through your primary care physician (PCP) and/or referring physician.

Imaging studies

If you would like to request copies of any imaging studies, (e.g. x-rays, MRIs) please contact the facility where you originally received those services.
